Return Request Form

To return an item please fill out this form completely and request an RMA#.  You will also be given an appropriate address to ship your product back with the RMA#.  Note all returns will be minus original shipping and a 15% restocking fee.  Items returned after 15 days from RMA# issued will not be accepted.  Please note, if a product with free shipping is returned for credit the cost of the original shipping will be deducted from your total credit.  If you are exchanging a product the buyer will be responsible for shipping both ways.
